Hip Replacement Surgery for Advanced AVN

When the femoral head has collapsed, replacement is not a defeat, it is the most reliable path back to a pain-free, active life. Here is what to honestly expect.

When is hip replacement the right answer?

In Grade 4 AVN, the ball of the hip joint (femoral head) has collapsed and arthritis has set in. At this point, no injection or drilling can rebuild the bone's shape. The damaged joint needs to be replaced.

Total Hip Replacement (THR) removes the damaged femoral head and the worn socket surface, and replaces them with smooth artificial components. It is one of the most successful surgeries in modern medicine.

  • Excellent pain relief: the deep AVN pain is typically gone right after surgery
  • Restored movement: walking, stairs, and daily activities come back step by step
  • Long-lasting: modern implants typically last 15–25 years
  • Proven: done successfully lakhs of times worldwide every year

But first: are you sure you need it?

If your AVN is Grade 1–3, do not rush into replacement. Many patients in India are advised replacement while their bone is still intact. If the femoral head has not collapsed, hip-preserving options like SVF therapy or core decompression with BMAC may save your natural joint, and replacement remains available later if ever needed. What the surgery involves

  1. Pre-surgery checks. Blood tests, X-rays, anesthesia fitness, and planning of the implant size.
  2. The operation. Usually 1–2 hours under spinal or general anesthesia. The damaged head is removed and the artificial ball and socket are fixed in place.
  3. Hospital stay. Typically 3–5 days. Most patients stand and take steps with a walker within 1–2 days.
  4. Going home. You leave with a physiotherapy plan, walking support, and clear do's and don'ts.

Recovery timeline: honest expectations

PeriodWhat to expect
Week 1–2Walking short distances with a walker. Wound care. Gentle physiotherapy begins.
Week 3–6Moving to a stick/cane. Longer walks. Stairs with support.
Month 2–3Walking unaided for most patients. Return to desk work and light activity.
Month 3–6Full recovery for most. Swimming, cycling, and travel are usually fine.

Precautions and lifestyle after THR

  • Avoid crossing your legs and deep squatting in the early weeks
  • Use raised chairs and western-style toilets initially
  • High-impact sports (running, jumping) are usually discouraged long-term
  • Keep a healthy weight to protect the implant
  • Attend follow-ups, implants should be checked periodically

Physiotherapy is a big part of a good result. See our guide to physiotherapy and supportive care.

Risks to discuss with your surgeon

THR is safe in experienced hands, but like all major surgery it has risks: infection, blood clots, dislocation in the early weeks, leg-length difference, and implant wear over the years. Choosing a certified facility and following the rehab plan keeps these risks low.

Hip Replacement: Frequently Asked Questions

How long does a hip replacement last?

Modern hip implants typically last 15 to 25 years, and many last even longer. For younger patients this matters, a replacement done at 35 may need a revision surgery later in life, which is one reason early-stage AVN patients should first explore hip-preserving options.

When is hip replacement the right choice for AVN?

When the femoral head has collapsed (Grade 4) or arthritis has set in, hip-preserving treatments can no longer help. At that point, total hip replacement is the most reliable way to remove pain and restore movement. Check the stages of AVN to understand grading.

How painful is recovery after hip replacement?

Most patients are surprised: the severe AVN pain is usually gone almost immediately after surgery, replaced by manageable surgical pain that fades over weeks. Most people walk with support within a day or two of surgery.

I have early-stage AVN. Should I just get a replacement and be done with it?

We advise against rushing. If your bone has not collapsed, hip-preserving treatments like SVF therapy or core decompression can save your natural joint, which no artificial implant fully matches. Replacement will still be available later if needed. Get a second opinion on your MRI first.

What precautions are needed after hip replacement?

For the first weeks: avoid crossing your legs, avoid deep squatting and low chairs, and follow your physiotherapy plan. Long term, most patients return to walking, swimming, cycling and normal daily life. High-impact sports are usually discouraged.
