AVN Symptoms: Early Signs of Avascular Necrosis in the Hip
Avascular Necrosis (AVN) means bone tissue dying because its blood supply has been cut off. The tricky part? The bone starts dying long before it starts hurting. Knowing the early signs, and acting on them, is often the difference between saving your hip and replacing it.
Early symptoms (Grades 1–2)
In the beginning, AVN whispers rather than shouts:
- Groin pain that comes and goes. A deep ache in the groin, roughly where your trouser pocket sits, is the classic first sign. It may also be felt in the buttock or outer hip.
- Pain after activity. A long walk, climbing stairs, or standing for hours brings on the ache. Rest settles it, so it is easy to dismiss.
- Occasional thigh or knee pain. Hip problems often send pain down the thigh. Some AVN patients first visit a doctor for "knee pain".
At this stage an X-ray is usually normal. Only an MRI scan can catch the disease, and this is exactly when treatment works best.
Progressing symptoms (Grades 3–4)
As the bone weakens and starts to collapse, symptoms become impossible to ignore:
- Pain at rest and at night. The ache no longer waits for activity. It can wake you from sleep.
- A noticeable limp. Your body automatically shortens the time you spend on the painful leg.
- Stiffness and reduced movement. Rotating the leg, spreading it outward, or lifting the knee becomes restricted.
- Trouble with Indian daily activities. Sitting cross-legged on the floor, squatting, and using Indian-style toilets become painful or impossible.
- Difficulty with stairs. Each step loads the damaged femoral head.

Why symptoms lag behind bone damage

Where exactly is AVN hip pain felt?
Most people feel AVN pain deep in the groin, roughly where a trouser pocket sits. It can also spread to the buttock, the side of the hip, or down the thigh towards the knee. Some patients first complain of knee pain, which is actually referred from the hip.
Can AVN be present with no symptoms at all?
Yes. In Grade 1, AVN often causes no pain. The bone damage is visible only on an MRI scan. That is why anyone with strong risk factors, long steroid use, heavy alcohol, sickle cell disease, should get screened if the other hip is affected or symptoms start.
How is AVN pain different from arthritis pain?
AVN usually starts in younger people (25–50), often affects one or both hips, and progresses over months. Common osteoarthritis usually starts after 55–60 and worsens over many years. AVN pain often includes rest and night pain earlier than arthritis. An MRI clearly separates the two.
Does AVN pain come and go?
In early stages, yes. Pain may appear after a long walk or activity and settle with rest. As the bone weakens, pain becomes more constant, and in later stages it disturbs sleep. Do not wait for constant pain, by then the disease is usually at a later grade.
When should I see a doctor urgently for hip pain?
See a specialist promptly if hip or groin pain lasts more than 2 weeks, wakes you at night, causes a limp, or you have risk factors like steroid use, heavy alcohol, or a past hip injury. An MRI at this point can catch AVN in a stage where the hip can still be saved with joint-preserving treatment.
